Exposure to nicotine solutions is dangerous because the e-liquids can contain highly concentrated amounts of nicotine. Nicotine poisoning from vaping and e-cigarettes can occur by ingestion (i.e. drinking e-liquids), inhalation (i.e., breathing in too many vapors) or absorption through the skin or eyes (i.e., direct contact with e-liquids).

Poison control centers in the U.S. have managed thousands of nicotine exposure cases relating to e-cigarette devices and liquid nicotine since 2011. Young children are at risk of nicotine poisoning from ingesting, swallowing, eating, drinking or even touching liquid nicotine. According to the CDC, more than half of the calls to poison centers due to e-cigarette poisoning involved children under the age of 5. According to the American Academy of Pediatrics (AAP), a small amount of liquid nicotine can kill a child and can be poisonous within minutes if spilled on the skin.

The amount of nicotine that can cause poisoning depends on, among other things, body weight, general health, the source of nicotine and other factors. According to the CDC, the fatal or lethal dose of nicotine for adults is estimated to be between 50 and 60 milligrams.

Signs & Symptoms of Nicotine Poisoning

Signs and symptoms of nicotine poisoning potentially may include:

  • Difficulty breathing
  • Vomitting
  • Increased saliva
  • Dizziness
  • Seizures
  • Change in blood pressure
  • Change in heart rate
  • Abdominal pain
  • Tremors or convulsions
  • Twitching
  • Lack of coordination
  • Headaches
  • Coma
  • Paralysis
  • Respiratory failure
  • Drowsiness
  • Cough
  • Confusion
  • Anxiety
  • Eye irritations (for eye exposures)
  • Agitation
  • Pallor or paleness
  • Sweating
  • Weakness or fatigue

Severe cases of toxicity (though rare) may lead to heart failure or death.

Symptoms related to nicotine overdose usually happen in two stages. The first stage of symptoms (about 15 minutes to an hour after exposure) typically are related to the stimulatory effects of nicotine. The second stage of symptoms (about 30 minutes to four hours after exposure) are related to the the depressor effects of nicotine.
